How to enroll:

 

1. Go to our contact page or click the button below.

2. Enter your information in the contact page. We must have an email address to contact you.

3. Type in your insurance information and the name of the service you are interested in into the subject line.

4. Tell us anything you want us to know in the message including best times for a phone call.

5. Lydia Gentry will email you to schedule a brief phone call with you about our services and to help you with enrollment.

6. Make sure to click the button and complete the electronic communication form and email that back to Lydia.
